Complete, sign and date the claim form for your program (claim forms for all programs can be found here) and send it to the following address, along with the Physician's "Super Bill" or a fully itemized statement of charges with diagnosis on the attending Physician's letter head or regular billing. (Make a copy for yourself.) Send to:
