USI Affinity Travel Insurance Services Apple iPad Giveaway Sweepstakes

1. Eligibility:

This Sweepstakes is offered only in the United States and void in Puerto Rico, outside the United States, and anywhere else prohibited by law. All participants must be 18 years of age to be eligible to participate. Participants must have a valid US mailing address. All current and former employees and directors of USI Affinity Travel Insurance Services, affiliated and subsidiary companies and their advertising and promotional agencies and the immediate family or members of the households of such employees and directors (whether or not related) are not eligible. By entering, you agree to these Official Rules and the decisions of Sponsor, which are final and binding in all respect.

2. To Enter:

To participate in the sweepstakes, simply enter your email address, click on “Subscribe Now," and click “Save Profile Changes.” You will then receive a confirmation email (if you are not already subscribed). By confirming your email subscription, you agree that you would like to receive our special Email Travel Alerts to keep you informed on our latest news. (You may opt-out of the receipt of such information at anytime by following the instructions in the email. However, opting out during the sweepstakes promotional period will forfeit your entry in the sweepstakes.) Only valid Email Subscribers as of June 1, 2010, will qualify for the drawing. For bonus entries, you must be a valid Twitter Follower and/or a valid Facebook Fan (now known as "People Who Like This"). (If you were already a Twitter Follower or a Facebook Fan prior to the start of the Sweepstakes, you are automatically qualified for a bonus entry only if you are also signed up for our newsletter as of June 1, 2010.)

3. Promotional Period:

Sweepstakes promotion begins on June 01, 2010 at 12:01:00 AM P.S.T. and ends on June 30, 2010 at 11:59:59 PM P.S.T. Winner will be announced on July 15, 2010. If a minimum of fifty entrants has not been met by June 30, 2010 at 11:59:59 PM P.S.T., the promotion will be extended until July 31, 2010 at 11:59:59 PM P.S.T., and the winner will be announced on August 16, 2010.

4. Email:

By participating in the sweepstakes and acknowledging the receipt of the confirmation email, you will opt-in to receive email or other communications from USI Affinity Travel Insurance Services. You may subsequently opt-out of the receipt of such email or communications by following the directions in the email or by contacting the site. Opting out during the Sweepstakes Promotional Period will forfeit your entry in the sweepstakes. However, you may choose to opt-out after the promotional period. During the sweepstakes period, however, you may nevertheless receive emails which USI Affinity Travel Insurance Servicesdeems are related to the Sweepstakes and of related programs if you are opted in.

5. Random Drawing:

The winner of the sweepstakes will be drawn at random after the sweepstakes ends.

6. 1 Grand Prize:

Apple iPad. The above mentioned prize will be sent to the winner of the sweepstakes upon the receipt of the completed and signed forms (distributed at the end of the drawing via email). Note: USI Affinity Travel Insurance Servicesreserves the right to substitute a prize of equal value if advertised prize is unavailable, as well as to add Bonus Entry Opportunities and/or additional Prize Opportunities at any time during the promotion.

7. Claiming Prize:

Winner will be announced and notification email will be sent out within 15 business days of the drawing result. Winners will be required to complete and return an Affidavit of Eligibility, a Liability Release, and where lawful, a Publicity Release, and other required information within 5 business days of notification. If winner fails to respond in a timely manner, or if prize notification or prize is returned as non-deliverable, or if winner is found not to be eligible or not in compliance with these Official Rules, disqualification will result and an alternate winner will be selected. All taxes, including federal, state and local taxes are the sole responsibility of the winner. If required by law, winner will be issued an IRS 1099 tax form (Winner is advised to seek independent counsel regarding prize winnings.)

8. General Conditions:

By accepting the prize, where permitted by law, winners grant to Sponsor, and those acting pursuant to Sponsor's authority (and agree to confirm said grant in writing) the right to use and publish his/her name, portrait, picture, voice and likeness for advertising, trade and promotional purposes without additional consideration, worldwide, and on the World Wide Web, without notification, review or approval. Further, participants release and agree to hold harmless Sponsor, its affiliates and their respective directors, officers, employees, representatives and agents from any and all liability for any injury, loss or damage of any kind to person, including death, and property, arising in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of the prize, participation in any Sweepstakes related activity, or participation in this Sweepstakes.

USI Affinity Travel Insurance Services reserves the right, at its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the sweepstakes or web site; to be acting in violation of the Official Rules; to be acting in violation of the terms of the web site, or to be acting in a non-sportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person. Any use of robotic, automatic, programmed or the like entry methods will void all such entries by such methods, and disqualify any entrant using any such method. In the event of a dispute as to entries submitted by multiple users having the same email account, the authorized subscriber of the email account used to enter the Sweepstakes at the actual time of entry will be deemed to be the participant and must comply with these rules. Authorized account subscriber is reckoned to be the natural person who is assigned an email address by an Internet access provider, on-line service provider or other organization which is responsible for assigning email addresses.
